GROUP OR INDIVIDUAL EVALUATION
The registration form has a drop-down selection where you can choose the test, location, & date for each child.
When requesting a PIAT-R or Portfolio Review it says “I will contact you for an appointment”. If you would like an in-person evaluation at a one of the group testing sites, put that in the comments section.
There is an option to pay with PayPal on the registration form.
That is optional. If you are paying later or another way, skip that section and click the “submit” botton below the payment section.

Note: If you do not get a “Thank You” page and an email receipt after completing the registration form please send a message to kitty@slusserlife.com with something like “I Registered” in the subject line and “no confirmation” in the body.
